A-Plus will be administering the Flu and Pneumococcal Shots. The Flu vaccine is available to anyone older than 6 months and the pneumococcal vaccine to anyone older than 13 years of age.
IMPORTANT Billing Information:
Anyone receiving a vaccine must present a valid form of Identification.
Medicare Recipients must provide their Medicare ID
A-Plus will bill Medicare directly, with no co payment or deductable. Medicare MUST be your primary insurance company; they will not pay if you have another insurance company. You must pay A-Plus and then submit a receipt to your insurance company to get reimbursed. A-Plus is unable to bill any other insurance companies.
Non-Medicare recipients will be charged $30.00 for Flu and $45.00 for Pneumococcal vaccines.
First come first serve, from 9 AM—4:30 PM
Forms are available at City Hall for anyone who wishes to fill them out prior to the clinic.
